CHES’ nursing support is available to patients who want self-directed services (SDS), which is a long-term mode of care where the patient makes the decisions on how, when, and where the service is delivered.
SDS offers more flexibility as compared to nursing homes or institutions. As part of this arrangement, nursing support is provided in the form of consultation, case management, and delegation.
The company’s consultation services include reviewing health risk screening tools (HRST), developing healthcare protocols, offering community health-related resources, and establishing lines of communication with healthcare providers.
CHES appoints registered nurses (RNs) who consult and work with the patient and their Primary Care Provider to deliver an individualized treatment plan attuned to their own needs. These patient plans are regularly reviewed by CHES – the team monitors the quality of the services provided with the goal to deliver optimal care.
While nursing support in the form of management and delegation deals with the provision of nursing-centered assessments and the development of care plans, it also includes the training, delegation, and supervision of staff assigned to carry out the services.
The nurses, in coordination with the Support Managers, will make arrangements with the caregivers who will ultimately provide the services to the patients.
CHES’ skilled specialists have experience working with individuals who have special needs. The staff is also specially trained to assist patients on the autism spectrum, as well as those with cerebral palsy and spina bifida.
